Description

This is a 2-day course consisting of a Lecture and a Practicum & Competency (P&C) session.

  • Day 1: Lecture
    All participants are required to attend the lecture on the first day. The lecture is always scheduled as the first session of the month.

Day 2: Practicum & Competency (P&C)

  • There are multiple P&C dates planned for each run. Each participant will attend only one P&C session. Each set of dates consists of a Lecture date and a P&C date. Please select only one set.
    For example, choosing 02/02/2026 – 04/02/2026 indicates that you will attend the Lecture on 2 February 2026 and the P&C session on 4 February 2026.

Learning Objective: This course is brought to you by Kwong Wai Shiu Hospital to enable staff from AAC/AACH/IBP and centres planning to be AAC/AACH to become a competent Community Screener assessor.

The Community Screener (CS) was developed to help identify persons who are presently or potentially at risk of health and social vulnerabilities. This allows the centre to pick up issues proactively and carry out timely intervention for seniors to address health and psychosocial issues, maximize functional capacity to enhance level of independence, and strengthen psychosocial well-being and safety net to live well and safe in the community.

Please note that this course is conducted solely in English.

Target Audience:

Active Ageing Centre (AAC) / AAC (Care) / IBP Staff assigned to conduct CS and to become certified CS assessors, with the following pre-requisites:

Education Level

GCE N-Level or higher

Language

Basic proficiency in both spoken and written English

Learning Outcomes (2 Days) :

1. Basic Gerontology [Day 1]: Learners to understand the common problems in the elderly guided by CS questions. Learners will be equipped with knowledge on the approaches to manage those problems in the elderly.

2. Application of the Community Screener (CS)[Day 1]: Learners to understand how to conduct the CS and provide appropriate care recommendations according to CS care path and own organization’s care protocol.

3. *4 Circuit Stations for Practicum and Competency assessment [Day 2]: Learners to achieve accreditation as a CS assessor with the ability to conduct CS proficiently and provide correct coding.

4. interRAI IT system [Day 2]: Learners to be able to complete the CS in the system, interpret the care path correctly and generate the required reports.

*There is a minimum of 1 day self-practice post-theory session prior to Practicum and Competency assessment.

Course Duration and Location:

The course will be held over 2 days at Kwong Wai Shiu Hospital.

  • Day 1: Theory
  • Day 2: Practicum (hands-on session) and Competency assessment.

Registration Details:

All learners would require LMS access to retrieve the training materials as well as complete the competency assessment submission.
Please make sure that all of the details in your learner profile is filled. This includes branch, as well as the billing person and their details. If you're unsure, check with your CCO admin. In the event that the CCO Admin registers on behalf of the learner, the CCO Admin must ensure that their learner can personally log in to AIC CCLMS with their SingPass at least once before the class in order to ensure that the learner can attend this CS Course successfully.

A minimum of 75% attendance (with CS theory and practicum mandated) and a pass in Competency assessment are required for the completion and of course and receive your certification.
Prevailing course fee subsidy at 90% for Singapore Citizens / Permanent Residents and 45% for Work Pass Holders working in eligible community care organizations.
