This one-day programme is designed to deepen participants’ understanding and application of the concepts and tools of incident management. The workshop will incorporate the principles of Root Cause Analysis (RCA) and Human Factors Analysis Classification System (HFACS), in line with guidelines from the Ministry of Health’s Framework on Serious Reportable Events.
Participants will learn how to:
1. Use RCA and the HFACS in analysing incidents.
2. Form and lead/facilitate effective teams to review and manage incidents;
3. Develop effective and actionable solutions to address findings from review and monitor the implementation of solutions

a. Role of RCA in Quality and Patient Safety
b. Understanding Human Factors and Incident Causation
c. Using HFACS for analysis
d. Review of the overall RCA Process (Steps 1-3)
e. Review of Overall RCA Process (Steps 4-6)
f. RCA Exercise

A minimum of 75% attendance will be required for you to complete the programme and receive your certificate.
Prevailing course fee subsidy at 90% for Singapore Citizens / Permanent Residents and 45% for Work Pass Holders working in eligible community care organisations.
Full fees apply for cancellations after email confirmation unless a valid reason is provided. Replacements are encouraged if withdrawal is necessary. If you/your staff need to withdraw from the course, please find a replacement.
