Description

This programme is brought to you by Kwong Wai Shiu Hospital to enable staff from the Community Care sector planning to become a competent Community Care Associate (CCA). The CCA will contribute to multi-disciplinary teams while continuing to deliver personal care. They will work alongside Nurses and Allied Health Professionals in delivering seamless and holistic care in Community Care settings. The programme seeks to further professionalise the Community Care sector and provide a clear progression pathway for existing care professionals who are looking to upskill themselves and guide prospective learners looking forward to a career in this sector. This programme will provide a comprehensive overview of skills and expertise one can expect as they journey towards becoming a Community Care professional. These will include domains such as Clinical Care, Care Coordination, Psychosocial and Therapy elements.

The course comprises of 2 main components:

b. On-the-job Training (OJT) On-the-Job Training (OJT) involves practical, hands-on learning experiences. The OJT Competency Logbook is a key document used during this training.
• The logbook contains competency checklists and is provided to learners.
• Once learners complete the tasks and receive their preceptor’s endorsement, the logbook should be submitted to the Training Providers. • Submission should occur no later than the last day of the CCA training programme.

Entry Criteria
A. Education: Obtained at least a C6 pass at GCE ’N’ level in any 1 subject
B. Language: WPLN 4 (Reading, Listening, Speaking) AND WPLN 3 (Writing)

• Active Aging Centres
• Day Rehabilitation Centres

Feeding Management in Community Care (Classroom & Asynchronous)
1. In LU1, learners will be introduced to the Types of Diets, including normal, therapeutic diets, and texture-modified diets. This segment introduces learners to the different dietary needs and provides context for supporting clients with specific nutritional needs in the subsequent units. 2. In LU2, learners progress to learning about a common condition in swallowing, Dysphagia. Dysphagia represents a critical feeding challenge. By learning about it immediately after different diets, learners can now relate how dysphagia impacts dietary needs. Introducing the signs and complication of Dysphagia helps learners recognise instances where the necessity for oral and tube feeding may be required. 3. In LU3, learners will be trained practically in proper Feeding Procedures for oral and tube feeding. By this stage, learners have a comprehensive understanding of diet types and dysphagia. They are now prepared to apply this knowledge in practical feeding procedures, ensuring compliance with standards to avoid any complications.

Infection Prevention and Control in Nursing Practice
1. Understand the fundamentals of infection control such as medical terminologies so that can better understand other healthcare professional. Knowing when and how to perform hand hygiene enables them to protect themselves as well as their patients 2. Understand what is standard and transmission-based precaution and how to follow the precaution such as choosing and wearing Personal Protection Equipment (PPE) according to patient’s condition 3. Identify and manage general and biohazard waste in accordance with standard and transmission-based precaution 4. Understand disinfection and sterilization methods in accordance to nature of equipment and the intended use to prevent transmission of diseases. 5. 1. Demonstrate competence in assisting with the administration of oral and topical medications, ensuring accurate dosage delivery and adherence to safety protocols. 2. Identify common drug allergy reactions, enabling prompt recognition and appropriate management to enhance patient safety and minimize adverse outcomes. 3. Adhere to proper protocols for medical storage and disposal, ensuring medication potency, preventing contamination and ensuring environmental safety.

1. Understand and monitor factors affecting mobility and ambulation so as to provide levels of assistance necessary for safe mobility and ambulation of patients. 2. Understand the safety and health procedures so as to apply the strategies and precautionary measures to prevent falls. 3. Understand the procedures and techniques of safe lifting, transferring and positioning of patients so as to perform safe lifting, transferring and positioning of stable patients in accordance with identified techniques and organisational policies. 4. Understand the concepts of safe mobility and ambulation so as to coordinate patients’ use of devices and ensure functioning of equipment to enable safe mobility and ambulation for patients.

1. Acknowledge the importance of understanding patients’ ability and mental condition, so as to deliver satisfactory person-centred care to patients. 2. Gain a good grasp of healthcare procedures, which are critical to perform care work well. 3. 1. Understand the importance of patient-centred practice and its impact on patient advocacy 2. In the realm of healthcare, healthcare workers are expected to acknowledge and value the client as an individual when providing care assistance. To set the foundation for respectful interactions, the module begins with a discussion on the appropriate actions to protect client’s privacy and dignity. This foundational topic serves as a guideline in all aspects of care assistance. Next, the module will progress from conceptual understanding to practical skills. In providing care assistance, learners will engage with clients facing chronic or acute health conditions. Learners should be adept at identifying common health conditions and understanding the guidelines for assisting clients with various health conditions. It is equally important for learners to acquire the knowledge of recognising abnormalities and potential risks in client conditions and reporting promptly to supervisors. Chronic health conditions can potentially lead to functional impairments, and hence the next objective is to prepare learners to assist clients with activities of daily living (ADLs). This involves the preparation needed before the session, for example, preparing the tools and communicating with clients.
