Patient Safety and Adverse Event Management
Course Description
This course provides participants with comprehensive knowledge and practical skills in the reporting an incident and managing adverse events across both clinical and non-clinical settings. The course will also cover definition of clinical incidents, learning from past incidents, incident review and management system, SAC (Severity Assessment Code) scoring, Human Factors Classification System (HFACS), RCA (Root Cause Analysis) to identify underlying causes of incidents, developing and implementing effective interventions, and monitoring their sustainability. Through practical exercises and case studies, participants will gain hands-on experience in conducting incident reviews, planning interventions, and ensuring continuous improvement in patient safety.
Effective communication is crucial in managing the aftermath of adverse events in healthcare settings. This course provides participants with the principles and practices of open disclosure, including strategies for delivering difficult news to families and managing their expectations. It also addresses supporting staff who may be considered "second victims" by applying the Just Culture approach during the review process. Key topics include defining incidents and communication, understanding roles and responsibilities, selecting appropriate communication channels, and utilizing communication templates. Through practical exercises and case studies, participants will develop the skills necessary to navigate complex conversations and foster a culture of transparency and support.

By the end of this 8 hour course, participants will be able to:
Understand principles of clinical audit and its unique purposes and scopes.
Design and conduct clinical audits to evaluate compliance with clinical care standards.
Analyse audit data to identify areas for improvement in clinical practice.
Develop and implement action plans based on audit findings to enhance patient safety and care quality.
Monitor and evaluate the effectiveness of interventions resulting from audit processes.
Foster a culture of continuous improvement through regular auditing and feedback mechanisms.
Collection, management and sharing clinical data
Setting performance targets and thresholds and understanding data trending.
