Course Description:
The Certificate of Achievement in Care Planning Facilitation recognises individuals who have successfully completed both the General Advance Care Planning (ACP) Facilitator Training and the Preferred Plan of Care (PPC) Facilitator Training.
This stacked learning pathway equips facilitators with the full spectrum of knowledge and skills to guide Advance Care Planning conversations across different stages of health and illness, from generally healthy individuals to those with progressive life-limiting or advanced illnesses.

General ACP Facilitator Training focuses on initiating and conducting ACP conversations with healthy individuals, applying the General ACP framework, and exploring the ethical, cultural, and procedural aspects of care planning.

PPC Facilitator Training builds on this foundation, enabling facilitators to work with patients facing serious or progressive illness, using the PPC framework to capture complex care preferences and ensure alignment with healthcare teams.
By completing both courses, participants will:

Understand the principles, goals, and scope of ACP in Singapore.

Apply both General ACP and PPC frameworks in appropriate contexts.

Demonstrate effective facilitation, communication, and documentation skills.

Create written care plans that are person-centred and actionable.
Certification:
Participants who pass the assessments for both modules will be awarded the Certificate of Achievement in Care Planning Facilitation. Healthcare professionals may be eligible for CPE points for each course completed.
Pathway Structure:
Course 1: General ACP Facilitator Training (11 hours)
Course 2: Preferred Plan of Care Facilitator Training (11 hours)
Completion of both modules is required for the COA award.
