Description

This 1.5-day course offers participants foundational knowledge and skills to serve as a social connector, bridging the gap between primary care and community resources. Participants will learn how to assist seniors in adhering to their health plans, empowering them to remain active and engaged in managing their health goals. The training aims to equip support staff and executives with the tools to support seniors in achieving better health outcomes through community-based interventions.

Course Objectives

At the end of the session, participants will be able to

  • Understand the concept of community as an ecosystem that support the seniors
  • Recognize one’s role in supporting Social and Lifestyle Interventions to seniors
  • Understand concepts related to Social Prescribing
  • Engage seniors in conversations through motivational interview techniques to understand needs and concerns
  • Interpret prescribed health plans to provide appropriate information and/or community referrals
  • Assist seniors in finding and booking activities that suit their needs
  • Report according to defined Standard Operating Procedures (SOPs)
  • Escalate cases which required specialized attention to appropriate stakeholders
