COURSE DESCRIPTION / AIMS

At the end of this course, participants will be able to:

  1. Understand the major geriatric conditions
  2. Understand the national health and age care initiatives
  3. Understand the age care landscape in Singapore
  4. Understand the archetypes of elders in the community
  5. Understand the key components and processes of geriatric case management
  6. Understand the screening and assessment tools required for older clients
  7. Understand the need and how to of developing outcome-based care plans
  8. Apply the necessary skills involved in case managing clients especially those with multiple needs (through applying the framework and approaches of case management to the analysis of real-world case studies)
  9. Work collaboratively within a multidisciplinary team to meet the needs of clients and their caregivers
  10. Understand the ethical issues surrounding case management of the geriatric population

COURSE OBJECTIVES / OUTLINE

  1. Understand the major health, functions and cognitive conditions of the geriatric population
  2. Understanding the What and Why of Case Management
  3. Understanding of the target population for case management
    • Clientele in need of CM
    • Risk assessment tools
  4. Roles and Functions of Case Managers
    • Key roles and functions - with emphasis on community case management (i.e., as opposed to acute or institutional case management)
    • Case managers’ networks
  5. Setting the Context – Introduction to the National Population Health, Healthier SG and Age Well SG Initiatives
  6. Understanding of the age care Landscape
  7. Understanding the Archetypes of Older Persons in the Community
    • Analysis of needs of case studies based on archetypes and matching with appropriate services
  8. Case Management Processes
    1. Case identification – Finding and screening
      • Review of agencies’ current practise
      • Understanding and applications of screening tools
    2. Admission and assessment
      • Review of agencies’ current practise
      • Understanding and applications of assessment tools
    3. Care plan development – focus on outcome-based care plans
      • Review of agencies’ current practise
      • Understanding the difference between outcomes and interventions
      • Development of outcome-based care plans for clients
      • Understanding outcome measurements
    4. Implementation of care plan
      • Review of agencies’ current practise
    5. Monitor, evaluate and advocate
