Integrated Care for Dementia is a blended course that aims to empower learners with the knowledge and skills to address and support multiple needs of persons living with dementia and caregivers across medical, psychosocial, and psychological domains. This is a contemporary care approach in enhancing the management of dementia and the wellbeing of the persons with dementia.

Course Objectives

• Apply Person-Centred Care approaches to care planning for persons living with dementia
• Understand health and neurological impairment
• Assess the needs of caregivers to provide information and referral to access relevant health and social services
• Recognise ethical principles, regulatory and legislations relevant to dementia
• Provide activities to maximise autonomy and promote wellbeing
• Develop and implement techniques which minimise the impact of changed behaviour

Course Content

This is a blended course. Learners are strongly encouraged to set aside adequate time to complete self-paced e-learning modules before attending classroom training sessions to fully appreciate the learning activities in class.

Assessment

At the end of the course, learners will be required to:

• complete 2 Written Assessments on the last day of the classroom facilitation.
• prepare a PowerPoint Presentation and complete a Practical Performance and Oral Questioning on 3^rd week after course.

Who Should Attend

Professional staff including nurses, allied health professionals, social workers, counsellors who wish to acquire entry level competencies to be a dementia practitioner, and those who develop care plans for persons living with dementia and support direct care staff to manage behaviours of concern.

SNB-CPE points will be awarded.

Trainers

Dr Ng Wai Chong, MBBS (Singapore) Grad Dip (Geriatric Medicine), is the Founder and CEO of NWC Longevity Practice, a social enterprise providing aged care consultancy, training, research, and direct clinical services. For over 20 years, Dr Ng has worked as a community aged-care physician. He is recognised as a thought-leader in healthy ageing – from clinical work, policy development and implementation to care system design. His areas of expertise are person-centred care, dementia care, frail care, end of life care, healthy ageing, population health and mindfulness training. He has been the chair and member of government committees including ElderShield Review Committee, Silver Industry Standards Committee, SPRING Singapore’s Technical Committee (Healthcare and Eldercare) and Vulnerable Adults Review Board. Dr Ng received the Minister for Health Award, Courage Fund Healthcare Humanity Award, and was named Global Ageing Influencer by Ageing Asia.

Dr Joy Lim is a clinical psychologist working with older adults, with a special interest in the area of dementia and depression. Joy has been actively involved in the assessment and intervention of people with dementia and their family/carers in home based, day centre and residential settings, as well as older adults with mental health issues. She is a Dementia Care Mapping (DCM) Advanced User (certified by the Bradford Dementia Group) and currently awaiting certification to become a certified trainer for DCM. Since 2012, Joy has acquired competence in conducting dementia care mapping for care quality assessment, monitoring and improvement, enriched model individual care planning, and care staff support and development. Joy is an ACTA certified trainer who is actively involved in facilitating adult learning for eldercare staff in the community, at hospitals and in nursing homes; lecturing on common mental health issues in older adults and specific topics on working with and engaging people with dementia.

Ms Cai Suqi is a registered social worker. She graduated from the National University of Singapore with Bachelor of Social Work and has acquired extensive experience in both acute and community settings. She has a strong interest in older adults and dementia. Suqi is the Programme Manager(Community Support Programme) of the Dementia Singapore Care Support Services and played a critical role in the development of the Eldersit Respite Care Services.
